Abstract

Lifestyle interventions are widely recognized for improving glycemic control and cardiovascular risk factors in Type 2 Diabetes Mellitus; however, their long-term effectiveness and sustainability remain uncertain.This study evaluated the comparative effectiveness of intensive lifestyle interventions versus routine care on glycemic regulation, cardiometabolic risk, behavioral adherence, and long-term sustainability. A mixed-methods experimental design combined longitudinal quantitative assessments of metabolic and cardiovascular outcomes with qualitative analysis of adherence and engagement behaviors. Participants were followed over extended periods to capture both early physiological responses and long-term outcome trajectories. Intensive lifestyle intervention was associated with significant reductions in glycated hemoglobin, insulin resistance, blood pressure variability, lipid dysregulation, inflammatory biomarkers, and composite cardiovascular risk indices compared with routine care. Anthropometric remodeling and improved metabolic stability were observed alongside strong correlations between adherence density and insulin sensitivity enhancement. However, sustainability analyses revealed heterogeneous long-term response patterns, with partial attenuation of benefits in some participants. Digital self-management tools enhanced engagement and scalability but demonstrated variable retention and diminishing effectiveness over time. Intensive lifestyle interventions yield substantial multidimensional benefits in Type 2 Diabetes management, though long-term durability depends on sustained behavioral engagement and adaptive support strategies. Integrating personalized maintenance frameworks and scalable digital solutions is essential for maximizing long-term cardiometabolic outcomes and informing future clinical guidelines.
